The visa application form, in two pages, was created in Microsoft Word and RTF formats.
Attention:   To print the proper visa application form, you must either have a copy of Office97 or Office2000 in your computer. The Consulate will not accept the visa application form printed incorrectly.

For Macintosh users, save the file of visa application form (visaform.doc) in a folder and, then, open it with Microsoft Word (for Macintosh).

It is recommended that the form be printed on an inkjet or laser printer.
