For VISA applicants under 18 years of age, both parents must sign the visa application form and have their signatures notarized. In addition to that, as well as the proper documentation for each applicable visa type, the following documents must be sent to the Consulate:

1. A notarized copy of the minor’s birth certificate;
2. If one or both parents are not accompanying the child during the entire trip, then a notarized travel authorization form  must be signed by the absent parent. Please click here to get the form;
