Special Requirements for Minors
For VISA applicants under 18 years of age, both parents must sign the visa application form and have their signatures notarized. In addition to that, as well as the proper documentation for each applicable visa type, the following documents must be sent to the Consulate:
- A notarized copy of the minor’s birth certificate;
- If one or both parents are not accompanying the child during the entire
trip, then a notarized travel authorization form must be signed by the
absent parent. Please click here to get the form;
- BRAZILIAN MINORS do not need a visa but they do need the travel authorization if both parents are not traveling with him/her. You can read the instructions for Brazilian minor here (in Portuguese)
VERY IMPORTANT: Please note that the notarization of documents issued in the US must be done by a Notary Public within the jurisdiction of this Consulate.
